Coexistance of Probable Neurosarcoidosis and Generalized Myasthenia Gravis: Case Report

Objective NA. Background Sarcoidosis is an immune mediated disorder characterized by the formation of non-necrotizing granulomas in multiple organs. The broad range manifestations sarcoidosis nervous system includes cranial neuropathy, meningitis or myelopathy. Generalized Myasthenia Gravis a neuromuscular antibodies against acetylcholine receptor, and typically presents with muscle weakness fatigue. Association Neurosarcoidosis infrequent. Design/Methods Results Case report: A thirty-five year old woman, natural from Senegal was admitted to hospital two month history left peripheral facial nerve palsy, progressive dysphonia dysphagia, associated fever, headache asthenia. Examination revealed, numbness (V VII nerve) right vocal cord paresis (X nerve). Chest ray showed bilateral hilar lymphadenopathy MRI revealed enhancement both Meckel cavum, trigeminal nerves canalicular segment nerve. CSF examination detected elevated protein lymphocytic pleocytosis. For further study PET/TC done which supradiaphragmatic infradiaphragmatic hypermetabolic adenopathies. Biopsy adenopathy performed objecting non-caseating consistent diagnosis probable neurosarcoidosis. Moreover, patient explained fatigue since she 18 years old, observed at proximal limb muscles. Acetylcholine receptor were electromyographic decremental response repeated stimulation. Conclusions It well known that autoimmune disorders may coexist some patients. are rare diseases different pathogenesis. Although their coexistance could be coincidental it also suggest immunologic mechanisms triggering occurrence these diagnoses together.
